Billing: Dr. Greenspan is an in-network provider for Medicare and BCBS of IL PPO. Beginning May 1st, 2023 our office will no longer submit out-of-network charges to Cigna, Aetna, etc. Those patients will need to pay in full at the time of telehealth or office visits (we'll require credit card information to be on file). Upon request, we can provide a CMS 1500 claim form for those patients to submit to out-of-network insurance. If you are seeing Dr. Greenspan in-network and have a co-payment, the co-payment must be paid at the time of your appointment. It’s important that you understand your insurance coverage, your yearly deductible and the possibility that you may have out-of-pocket costs after insurance. Please be aware that we expect any balance to be paid in full upon receipt of your statement. If you’re unable to cover the balance in full we encourage you to discuss this with us immediately as we may need to help you find more affordable care.
There will be a $20.00 late fee for any balance that's your responsibility and remains unpaid after 30 days.
If you have any questions or concerns regarding fees and/or billing, please don’t hesitate to bring them to our attention.
Information needed before your first appointment: Please complete all registration forms and provide a current insurance card as well as a copy of your driver’s license before your first appointment if possible (and inform us of any changes over time). Please see the “Forms” page to download the forms you’ll need to complete for our office. If you forget to do so, please arrive at our office early to complete those forms.
Appointments: Please be aware that your appointment is a reservation of Dr. Greenspan’s time. If you cancel late or miss your appointment for a non-emergency reason, you’ll be charged. The usual fee will be 100% for a no-show (an appointment not cancelled at least 24-hours in advance, with the exception of an emergency). An appointment cancelled during that appointment will be considered a no-show. An appointment cancelled less than 12 hours before the appointment will be considered a late cancellation, and a 50% charge will be assessed (again, with the exception of an emergency). It’s important to note that insurance won’t cover these charges, and they must be paid before your next appointment.
Also, be aware that if you arrive late to your appointment you’ll be given the time remaining in that appointment (the appointment cannot be extended and delay Dr. Greenspan's next appointment).
Please try to schedule your next appointment at the time of your current one rather than having your prescription request denied because you haven’t been in in a reasonable time frame. Patients doing well who are stable on medication must still see Dr. Greenspan within 6 (six) months.
Prescriptions: Dr. Greenspan typically fills prescription requests between 9 am and 4 pm Mondays, Tuesdays and Thursdays and between 9 am and 1 pm Wednesdays and Fridays (with the exception of holidays when the office is closed). Dr. Greenspan doesn't fill prescriptions on weekends. Please allow 24-hours for prescriptions to be filled. Our preference is that you request refills through your pharmacy so we can respond to electronic requests sent to us. If, however, you’re requesting a refill outside of Dr. Greenspan’s electronic prescribing program please leave your name, the name of the prescription(s), dose(s) and the pharmacy’s name, address and phone number. Please note that if you haven't seen Dr. Greenspan in over six months he may not fill your prescription without a new in-person or telehealth appointment.
Prior Authorizations: As you may know, insurance can ask a physician to participate in this extensive review process in order cover the cost of a brand name prescription (and even non-insurance-preferred generic medications). If Dr. Greenspan recommends one of these medications, he’ll spend whatever time is required to complete this process. If, however, you simply want a branded drug without a prior history of ineffectiveness or intolerance to the generic, there will be forty dollar charge for completion of a Prior Authorization.
HIPAA: Our office works diligently to comply with the Health Insurance Portability and Accountability Act. We have information in our office that outlines the rules of HIPAA, and you are welcome to take a copy with you. We ask that you complete a form that allows us to provide information to your insurance for payment and your pharmacy for prescriptions. Other than those disclosures, we request that you complete an individual release of information if you’d like us to call or send records on your behalf.
Telehealth, Phone Visits and Masking: Please review and sign our Telehealth form if you’ll be utilizing this format, but understand that Dr. Greenspan prefers to see patients in his office. If you’re fully vaccinated (as we are) we encourage you to come to the office, but please do so masked in order to protect vulnerable patients. If you haven’t been fully vaccinated, you’ll need to remain masked and be socially distant in our office. Those adamantly opposed to being vaccinated are welcome to discuss this with us, but we reserve the right to request that you obtain your psychiatric care elsewhere given the continuing risks you may pose to others. If you’ve permanently moved out of town you may request telehealth appointments on an interim basis until you can find care in you new locale. Please understand, though, that we won’t continue this on a long-term basis. We understand that some may not be able to come into the office and aren’t able to arrange telehealth appointments due to technological challenges. If so, you may request a phone session. These will be done while insurance supports them.
